If you have received a Direct Certification Letter from the school, you do NOT fill out a form. If all children were not listed on the notice please contact Michelle Stoffer 740-545-5301.
You must fill out a new form every year. The form only needs filled out once per family not for every student.
If your income changes, you can update the form at any time.
Three application options:
1. Free and reduced application is available online with your back to school forms on OneView. You will need to create a Payschools account and will also need the students ID number. If you are already using Payschools for lunch payments it will be the same log in information and will not need the students ID number, as your student is already linked to that account. If you need to add a new student to that account you will need the student’s ID. Contact the Building Sectretary for your student ID.
