Parents of kindergarten students:
You were given a list of
the shots that your child needs at kindergarten registration. For your convenience,
we are also providing the list below. Be sure to send in a copy of the updated shot
record to Mrs. Bond at the elementary school.
5-DPT
4-Polio
3-Hepatitis B
2-MMR
2-Varicella
Parents of 7th grade students:
All
seventh grade students are required to have a Tdap and a meningococcal
(Meningitis) vaccine before school starts. These vaccines may be given by
your family doctor, Maternal and Child Health Clinic or some local pharmacies.
Be sure to send in a copy of the updated shot record or copy of the receipt to
Mrs. Bond at the middle school.
Parents of 12th Grade Students:
All 12th grade students
are required to have a meningococcal (Menigitis) vaccine before they start
school. This is in addition to the meningitis vaccine that they may have had in
seventh grade. Be sure to send in the updated shot record to Mrs. Bond at the
high school. This also includes post secondary and career center
students.
